Ms. D’Arezzo, a Licensed Practitioner of the Healing Arts with a master’s degree in holistic counseling, has extensive experience working with children, teens, their families, and adults with anxiety, depression, mood disorders, behavioral dysregulation, ADHD, and significant traumatic histories.
Ms. D'Arezzo has advanced training in EMDR. She can provide in-person and online rapid responses, or early interventions, conduct fieldwork, and treat individuals and groups living recent, present, or past acute or prolonged adverse experiences. She also offers equine assisted psychotherapy (EAP) and equine assisted EMDR, EA-EMDR, — both appropriate for individuals and groups.

EQUINE ASSISTED PSYCHOTHERAPY &
EquiLateral: The Equine Assisted EMDR Protocol
Equine-assisted psychotherapy is a type of experiential mental health treatment that involves a person in therapy interacting with horses. Appropriate for ages seven and up. Equine-assisted psychotherapy has been shown to treat a wide range of mental health difficulties.
Concierge therapy
Concierge medicine is a type of medical practice in which patients pay an annual or monthly fixed fee or “retainer” to ensure more attention, contact, and thoroughness from their practitioners.
Concierge therapy is offered to a limited number of individuals, and families, at a time — this allows the practitioner a smaller patient caseload, thus more immediate and flexible scheduling, increased availability of the practitioner via phone or email, longer appointment times, and even home visits. Concierge therapy offers a number of benefits that traditional therapy does not afford. Use your monthly retainer’s hours as you see fit, and whenever you want.
Personalization
You may need a 15 minute chat, 30 minute check-in, or a two hour session — the time is yours to use as you need. Insurance companies will not dictate how many sessions you may have, nor for how long.
Meet via secure video chat or telephone, allowing you to have therapy wherever you are. If you are unsure if your needs are appropriate for teletherapy, that may be discussed and evaluated.

Individual therapy: $200 - $300 per 45 - 60 minute session; assessments $275. There is an addition charge of $85-100 for Equine Assisted Psychotherapy. The additional charge for equine therapy is payable directly to “the horses”. Payment is due at time of service. Insurance is accepted — BCBS and Tufts. I am not an in-network provider with any other insurance companies. For clients with out-of-network benefits, upon request, I will provide a “superbly”, or receipt of services. You use this to seek reimbursement from your insurance company. Submitting a superbill requires Ms. D’Arezzo to give you a diagnosis, which will become part of your medical record. Additionally, your insurance may ask for your therapy records to support reimbursement. If you wish absolute confidentiality, this may be unwelcomed. Please check your coverage.
In addition, Ms. D'Arezzo, was voted "Best Kids' Yoga Teacher", by LA Weekly, and was founder and director of Kids’ Yoga Circle. She is a frequently sought after subject expert, educator, and writer as well as a pioneer in the field of yoga for children with special needs and mental health issues. Through Kids' Yoga Circle, Ms. D’Arezzo conducts kids’ classes and workshops, family retreats, and teacher trainings worldwide. Kids' Yoga Circle's Reach Out Program has been implemented in schools, youth centers, and children's hospitals.
As a former contributing editor and writer for Yogi Times magazine, Ms. D’Arezzo has written more than a dozen "Kids on the Mat" columns on such topics as yoga and ADHD, childhood obesity, and childhood sleep disorders.
Ms. D’Arezzo began teaching in 1999 in some of the Los Angeles area’s most celebrated yoga studios. Darlene currently teaches in the tradition of T. Krishnamacharya, having studied with Krishnamacharya's grandson, and with Gary Kraftsow and the American Viniyoga Institute. Ms. D’Arezzo also studied with Mr. Iyengar in Pune, India.
PRACTICE POLICIES
These policies are in force for my practice to remain operational and for me to be able to continue serving all of my clients.
APPOINTMENTS, CANCELLATIONS AND “NO SHOWS”
I recommend that you verify coverage and patient responsibility before each session.
You must fulfill any treatment authorization requirements.
You are responsible for all charges, regardless of insurance coverage.
You must notify this office of any changes in your health insurance coverage and/or co-payments immediately, to ensure proper filing, etc.
You must pay any co-payments, co-insurance, and/or balances at the time of each appointment.
If you refuse to pay your bill, we reserve the right to use an attorney or collection agency in order to secure payment.
If you refuse to pay your bill, you agree to pay all collection fees, and attorney fees, necessary for the collection of any debt. Accounts 90 days in arrears will be turned over for collections and reported to your insurance company.
Once we schedule an appointment, two full business days notice, irrespective of weekends, must be given if you need to cancel or re-schedule. Otherwise, you are responsible for paying the full cost ($125) of the time that has been reserved for you. This is necessary because a time commitment is made to you and is held exclusively for you.
If you are late for a session, you may lose some of that session time. If you are so late that a claim may not submitted, you must pay for “no show”/cancelation fee.
Please remember to cancel and/or re-schedule 48 hours in advance. You are responsible for the time set aside for you if cancellation (or “NO SHOW”) is less than 48 hours, irrespective of weekends. A minimum of $125 is charged TO YOUR CARD ON FILE as a late cancellation and/or “no show” fee following a missed session. Should you dispute this charge, you forfeit your right to privacy. To defend my business, against this wrongful dispute, I share your signed financial agreements with the credit card claim, or bank claim, to any investigator.
Your appointment may be re-scheduled once the late cancellation and/or“no-show” fee has been paid. Insurance will not pay for missed sessions, and I cannot be responsible for covering the cost of sessions that are missed due to circumstances beyond my control. These include, but are not limited to, illness, child’s illness, last-minute changes in your work schedule, lack of transportation, missed or late busses, and the actions of others (such as a childcare provider), or other such reasons.
The standard meeting time for psychotherapy is 38 minutes to 53 minutes minutes. It is up to you, however, to determine the length of time of your sessions. Requests to lengthen the “45 minute” or “60 minute session” needs to be discussed with the therapist in order for time to be scheduled in advance. You must also contact your insurance company to determine eligibility and any co-pays or co-insurance. Ask your insurance company if a 90 minute individual therapy session with add-on code 99354 is allowed and authorized.
You are responsible for paying any co-pays or co-insurance at the time services are provided. The fee for an initial session, which usually lasts approximately 50-60 minutes, is $300.00. The fee for follow-up sessions, which last 38 - 53 minutes, is $200-250. Please confirm your co-pay and/or co-insurance before scheduling an appointment.
Additional sessions will not be scheduled until any outstanding fees and payments are received.
If your therapist is required to provide any documentation, letters, courtroom documentation, etc. or verbal testimonial, the fee is $325 per hour; one hour minimum. Payment is made in advance.
Any overdue balance will be charged a $35.00 late fee per month until paid in full. If you refuse to pay your bill, we reserve the right to use an attorney and/or collection agency in order to secure payment. You are responsible for any fees incurred by this office to secure your debt.
Payment is made with credit card on file. Should a check be accepted, a $35.00 service charge will be charged for any checks returned for any reason for special handling.
DOCUMENTATION and INTIAL SESSION
Any initial documentation must be completed 48 hours before your initial scheduled session. If I am to submit claims to your insurance company, you must upload your insurance information with this documentation. If you need additional time to complete the paperwork, please contact the office. If you fail to complete paperwork, your initial session will be canceled and you will be charged for the time set aside for you — $300.

TELEHEALTH
Please note that face-to-face sessions are highly preferable to phone or video sessions. However, in the event that you are out of town, sick, or need additional support, phone sessions are available. If a true emergency situation arises, please call 911 or any local emergency room.
SOCIAL MEDIA AND TELECOMMUNICATION
I maintain a professional identity on social media. I do not attract, or connect, and limit interaction with potential or current clients and colleagues on social media. Due to the importance of your confidentiality and the importance of minimizing dual relationships, I do not accept friend or contact requests from current or former clients on any social networking site (Facebook, Instagram, LinkedIn, etc.). If you message me via social media, I will not respond. I believe that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our therapeutic relationship. If you have questions about this, please bring them up when we meet and we can talk more about it.
ELECTRONIC COMMUNICATION I cannot ensure the confidentiality of any form of communication through electronic media, including text messages. If you prefer to communicate via email or text messaging for issues regarding scheduling or cancellations, I will do so. While I may try to return messages in a timely manner, I cannot guarantee immediate response and request that you do not use these methods of communication to discuss therapeutic content and/or request assistance for emergencies.
Services by electronic means, including but not limited to telephone communication, the Internet, facsimile machines, and e-mail is considered telemedicine by the State of California. Under the California Telemedicine Act of 1996, telemedicine is broadly defined as the use of information technology to deliver medical services and information from one location to another. If you and your therapist chose to use information technology for some or all of your treatment, you need to understand that: (1) You retain the option to withhold or withdraw consent at any time without affecting the right to future care or treatment or risking the loss or withdrawal of any program benefits to which you would otherwise be entitled. (2) All existing confidentiality protections are equally applicable. (3) Your access to all medical information transmitted during a telemedicine consultation is guaranteed, and copies of this information are available for a reasonable fee. (4) Dissemination of any of your identifiable images or information from the telemedicine interaction to researchers or other entities shall not occur without your consent. (5) There are potential risks, consequences, and benefits of telemedicine. Potential benefits include, but are not limited to improved communication capabilities, providing convenient access to up-to-date information, consultations, support, reduced costs, improved quality, change in the conditions of practice, improved access to therapy, better continuity of care, and reduction of lost work time and travel costs. Effective therapy is often facilitated when the therapist gathers within a session or a series of sessions, a multitude of observations, information, and experiences about the client. Therapists may make clinical assessments, diagnosis, and interventions based not only on direct verbal or auditory communications, written reports, and third person consultations, but also from direct visual and olfactory observations, information, and experiences. When using information technology in therapy services, potential risks include, but are not limited to the therapist's inability to make visual and olfactory observations of clinically or therapeutically potentially relevant issues such as: your physical condition including deformities, apparent height and weight, body type, attractiveness relative to social and cultural norms or standards, gait and motor coordination, posture, work speed, any noteworthy mannerism or gestures, physical or medical conditions including bruises or injuries, basic grooming and hygiene including appropriateness of dress, eye contact (including any changes in the previously listed issues), sex, chronological and apparent age, ethnicity, facial and body language, and congruence of language and facial or bodily expression. Potential consequences thus include the therapist not being aware of what he or she would consider important information, that you may not recognize as significant to present verbally the therapist.
(6) Telehealth sessions may not be recorded.
MINORS If you are a minor, your parents may be legally entitled to some information about your therapy. I will discuss with you and your parents what information is appropriate for them to receive and which issues are more appropriately kept confidential.
TERMINATION Ending relationships can be difficult. Therefore, it is important to have a termination process in order to achieve some closure. The appropriate length of the termination depends on the length and intensity of the treatment. I may terminate treatment after appropriate discussion with you, and a termination process, if I determine that the psychotherapy is not being effectively used or if you are in default on payment. I will not terminate the therapeutic relationship without first discussing and exploring the reasons and purpose of terminating. If therapy is terminated for any reason or you request another therapist, I will provide you with a list of qualified psychotherapists to treat you, upon your request. You may also choose someone on your own or from another referral source.
Should you fail to schedule an appointment for three consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, I must consider the professional relationship discontinued.
COMPLAINTS
If you are unhappy with what is happening in your therapy, I hope that you will discuss this with me as your therapist so that I can respond to your concerns. Your criticism will be taken with care and respect. If you believe that your therapist is unable or unwilling to listen, you may contact the State of Rhode Island: Department of Health.
You have the right to considerate, respectful and safe care without discrimination as to race, ethnicity, color, gender, sexual orientation, age, religion or national origin. You have the right to ask questions of your therapist regarding your therapy and your therapist’s training or experience. You have the right to request a referral to another therapist and you have the right to end your therapy at any time.
